Horatio Alger Transcript Release Form

This form authorizes UNCW and the Office of the Registrar to release official transcripts for the requestor indicated below. This form is only for use by students visiting UNCW through an agreement between UNCW and the Horatio Alger Association of Distinguished Americans, Inc.

This transcript issued includes all coursework attempted at UNCW.

Per the agreement, UNCW will issue one official transcript to the student at the mailing address indicated below and one official transcript to the student’s home institution as indicated below. These transcripts are sent at no cost to the student. Any additional UNCW transcripts needed will need to be requested and paid for by the student through the university’s normal transcript ordering system.

The UNCW transcripts will be released upon the close of grading and end-of-term processing.
