Inclusive Futures Program Application

Thank you for your interest in the Inclusive Futures Program. While the culmination of successfully completing all requirements of the Inclusive Futures Program is earning the Inclusive Life Skills and Career Development (ILSCD) Certificate, the ILSCD Certificate is currently pending approval through the university’s curriculum approval process. We fully expect this credential to be approved but you are applying for the Inclusive Futures Program without a program credential. 

The below requirements must be submitted to be considered for the program: 

1. Complete the application below.

2. Once your application is submitted, you will receive an emailed link to the applicant portal. Here you will be prompted to upload the following documents:

• Unofficial school transcripts (last 5 years) 
• Personal letter of recommendation
• Letter of interest (written or video, explaining your interest in UNCW/IFP) 

3. Submit contact information for Professional Recommendation

4. Send documentation of an intellectual disability, autism, and/or multiple disabilities to IFP@uncw.edu (e.g., IEP, psychological assessment, or medical report). 

5. A parent/guardian must complete the Parent/Guardian Assessment here: https://forms.office.com/r/uFM4scAiEe
